Lost Payment Transactions question

For those that take payments whether online or in person do any of you ever have a situation where transactions are ever lost? If so, is this something that happens weekly, monthly, or very rarely? I am trying to gather some intelligence on this from merchants, any help you can all provide would be great!! Thanks
