Statement of agreement

  My business plan calls for subscriptions.  Some of these will be person to person and other will be over the phone.
 
  I am concerned about how to go about setting up a statement of agreement to set up automatic bill pay, and how to go about applying the agreement over the phone.
 
  Any suggestions.
