Here are a few U.S.-based manufacturers I know of. I don't know many details regarding licensing/labels with any of these companies, though:
Kelii Flip Flops
Okabashi Brands, Inc.
Gurkee's (manufactures "rope sandals")
Chaco (customized pairs made in Michigan)
